According to the Royal Malaysian Navy board of enquiry findings the initial responses to the fire aboard RMN Sri Inderapura were inadequate. The fire broke out early on 8th October and restarted on 9th October. The ship was so badly damaged that it is to be decommissioned. Electrical short circuit was considered the most likely cause of the fire.
